On March 25, 2024, Alluvion Health in Great Falls announced the revitalization of its Project in Assistance in Transition from Homelessness (PATH) program with a new team of professionals. The PATH program focuses on supporting individuals experiencing homelessness, especially those dealing with mental illnesses or substance use disorders.

The program provides a range of services to its clients, including outreach, engagement, case management, and service coordination. By identifying eligible individuals and assessing their needs thoroughly, the PATH program aims to connect them to the necessary resources that will facilitate their transition out of homelessness.

Alluvion Health is committed to upholding the dignity, privacy, and confidentiality of all patients and their families through the PATH program. By taking a person-centered approach, the program aims to create a supportive environment that fosters recovery and stability for those experiencing homelessness.

Kevin Purinton, PATH Program Manager, stated that addressing healthcare disparities is at the forefront of Alluvion Health’s commitment to making a significant impact on the lives of those facing mental health challenges and homelessness. By offering comprehensive support to individuals in need, Alluvion Health hopes to improve their well-being and contribute positively to the community.
